Cancellation Policy

Dr. Fusion IV Therapy & Wellness We understand that plans can change. To provide the best possible service to all of our patients and maintain appointment availability, **Dr. Fusion IV Therapy & Wellness** requires at least **24 hours’ notice** for appointment cancellations or rescheduling. Cancellations & Rescheduling Appointments cancelled or rescheduled **less than 24 hours before the scheduled appointment time** are subject to a **$50 cancellation fee** and **forfeiture of any deposit paid** toward the appointment. No-Show Policy Patients who fail to arrive for a scheduled appointment without prior notice will be considered a **no-show** and will be subject to a **$50 no-show fee**, in addition to **forfeiture of any deposit paid**. Late Arrivals Arriving late may reduce the amount of time available for your appointment. Depending on the circumstances and our schedule, your appointment may need to be shortened or rescheduled. If the appointment must be rescheduled due to a late arrival, the cancellation policy may apply. Emergencies We understand that unexpected emergencies can occur. Exceptions to this policy may be considered at the discretion of **Dr. Fusion IV Therapy & Wellness**. Services must be paid in full at the time of services Health insurance typically does not cover services provided at Dr. Fusion LLC. If you want to seek insurance reimbursement, we would be happy to provide you itemized invoices that you can submit to your insurance company. By scheduling an appointment, you acknowledge and agree to this Cancellation & No-Show Policy. Thank you for respecting our time and the time of other patients.
