You may be suject to a $50 fee if cancelation or rescheduling is not done 24hrs prior to your appointment time. ________Services must be paid in full at the time of service. ________Health insurance typically does not cover services provided at Dr. Fusion LLC. If you want to seek insurance reimbursement, we would be happy to provide you itemized invoices that you can submit to your insurance company.
